Appointments

All appointments must be scheduled in advance. If you need to reschedule or cancel, please provide a minimum of 24 hours’ notice to avoid a cancellation fee.

Insurance

St Mary Open MRI accepts most insurance plans. Please check with your insurance provider to confirm your coverage before your appointment. Patients are responsible for any co-payments or deductible amounts at the time of their appointment.

Payment

Patients are responsible for payment of services rendered. St Mary Open MRI accepts cash, checks, and major credit cards.

Refunds

Refunds will only be issued in the case of a billing error or incorrect charge. No refunds will be given for services rendered.

Privacy

St Mary Open MRI takes the privacy of its patients seriously and follows all HIPAA regulations. Personal and medical information will not be shared with any third party without the patient’s written consent.

Medical Information

Patients are responsible for providing accurate and complete medical information at the time of their appointment. St Mary Open MRI will not be held responsible for any adverse outcomes as a result of incorrect or incomplete information provided by the patient.

Limitation of Liability

St Mary Open MRI will not be held responsible for any damages or losses resulting from the use of its services. Patients waive the right to take legal action against St Mary Open MRI in the event of an adverse outcome.

Changes to Terms

St Mary Open MRI reserves the right to make changes to these terms and conditions at any time without prior notice.

Governing Law

These terms and conditions are governed by the laws of the state of Indiana and any applicable federal laws.
